As a licensed Durable Medical Equipment provider, Heavenly Tubs may collect limited health-related information in the course of assisting customers with Medicare eligibility verification and reimbursement claims. What Information We May Collect

  • Name, date of birth, and contact information
  • Medicare beneficiary number
  • Healthcare provider name and prescription/Certificate of Medical Necessity
  • Diagnosis or condition information as documented by your healthcare provider

How We Use This Information

Health-related information is used solely for the purpose of processing Medicare reimbursement claims and verifying DME eligibility. We do not use health information for marketing purposes.
