As a licensed Durable Medical Equipment provider, Heavenly Tubs may collect limited health-related information in the course of assisting customers with Medicare eligibility verification and reimbursement claims. - Medicare beneficiary number
- Healthcare provider name and prescription/Certificate of Medical Necessity
- Diagnosis or condition information as documented by your healthcare provider

Health-related information is used solely for the purpose of processing Medicare reimbursement claims and verifying DME eligibility. We do not use health information for marketing purposes.
